Bariatric surgery is the most effective obesity treatment, but diabetes is an absolute contraindication. Little is known about the effect of type 2 diabetes mellitus (T2DM) on the long-term survival after bariatric surgery. This study investigated the effect of T2DM on the risk of death in patients undergoing bariatric surgery. We used a nationwide population-based cohort study of patients undergoing a first-time bariatric surgery in Denmark from 1995 to 2012. We excluded patients with T1DM and with a history of cancer before or after the bariatric procedure. We estimated the hazard ratios (HRs) of mortality using Cox regression. The study population consisted of 5653 patients, of whom 314 (5.5%) had T2DM. In the entire cohort, the mortality rate was reduced from 3.8% after 1 year to 1.0% after 10 years, and there was a trend of improved survival among patients with T2DM compared with those without T2DM. The risk of death did not differ between patients with and without T2DM (HR 1.01, 95% confidence interval 0.80-1.27). After adjusting for the main confounders, including baseline body mass index, age, and the Charlson comorbidity index, the risk of death was similar in patients with and without T2DM (HR 1.07, 95% confidence interval 0.80-1.43). The mortality rates in patients undergoing bariatric surgery are similar in those with and without T2DM. Further studies are needed to investigate the effect of diabetes on the long-term survival after bariatric surgery.Q:
